How to be “whole beings” rather than simply physical people

Many clients and people walk through our doors hoping to be touched by the “personal training fairy” and walk out with a perfect body 6 weeks later. The reason this happens is that the popular media sets an impossible standard of physical appearance and convinces people that with the right help they can achieve it. The real problem here, as I see it, is not as much the impossible standards as the fact that fitness popularly has been about physical good looks and not much else. This sets people up for false beliefs and a failure approach for sure. As professionals we know better and need to send the Right message and take a stand.

Simply put, people exercise to look good. Leaving out two other missing links!

This is backward thinking. To deal with it, I use an approach I call “in and out fitness.” In and out fitness is all about remembering the real person inside the body, valuing that person and teaching him or her to value himself or herself enough to maintain a healthy, active lifestyle. This method has been an effective tool in getting my clients to understand what fitness really is so they not only become active but also learn to enjoy a healthy lifestyle and to get better understanding about Lifestyle changes versus just a one time quick outer change.

Most of my clients have health issues, and many are ashamed of what they look like. The first and most important thing that I do when I meet new clients is to simply accept them for who they are without any preconceived ideas. In other words, I try to “see them from the inside out.” I know this better than anyone being an obese child myself. Judging people without knowing their personal life story is not right. Why people look and feel the way they do is typically a deeper issue than we realize.

In our first phone call or meeting, I strive to find out something about them I can compliment. I want to reassure them that they are not the sum total of their looks. I want them to know that if they view exercise as a way to become healthy instead of just a strategy to look better, weight loss and good looks will often be by-products of exercise. I do what I can to make them feel comfortable with me. I am genuinely interested when I talk to them about their issues and goals. I then address any misconceptions that they may have about fitness. I use the professional resources available to me to dispel any myths they may have and try to interpret these resources in a way they can understand. (This can be different for each individual.)

I want my clients to understand what they need to do to meet their goals and to think about redefining those goals if necessary, based on what they are actually willing to do. I believe that honesty goes a long way in developing relationships with clients. Most people appreciate this honesty. Creating achievable goals step by step and asking proper questions helps me to set up open, honest, and winning coaching to help my clients feel empowered to reach their potential.

Another thing my clients appreciate is my attention to their preferences. During our initial exercise sessions I carefully observe and ask questions to determine their “exercise personality.” Do they like vigorous work or are they more low-key? What philosophy do they live by, and how will it affect their exercise and eating habits? How busy is their life? What are their favorite activities and music? With this information, I can design a program that will be not only as safe and effective as possible but also interesting enough for them to want to follow.

An exercise program is only successful if the client adheres to it. For example, I have a client who only likes to work out in natural light. When she comes in, I turn off the lights in the room and open the blinds. I have another client who loves using exercise balls only because he feels safe, so I make sure we only use balls during his training sessions and I bring educational articles on ball exercises with me for his appointments. If a client likes specific music I bring them their prefered music and create a theme for the workout. For some clients, I ask what kind of day they’ve had and make sure the program I have created is flexible enough to accommodate bad ones. I may offer a choice of two or three different exercises or programs a client can do that day. For other clients, I always have a set program, so the only thing they need to think about is performing each exercise correctly. Personalizing the program and approach helps people enjoy working out and, more importantly, feel valued. Valuing clients keeps clients, and we all know how important that is in this business. The more Personalized the better after all these are human beings and the influence we have in their lives can be very great!

Why You need to be aware of Cancer risks?



End-of-Life Conversations Lower Costs, Improve Quality of Life

Advanced cancer patients who have end-of-life discussions with their doctors appear to have better quality of life in their final days and pay significantly less for that care, according to a new study published in the Archives of Internal Medicine.

The research is based on data from the ongoing Coping With Cancer study, a large multi-site study tracking the health of advanced cancer patients and their primary caregivers. It's being funded by the National Institute of Mental Health and the National Cancer Institute, and incorporates work from several research centers, including the Dana-Farber Cancer Institute, Yale University, Memorial Sloan-Kettering, and the University of Texas Southwestern Cancer Centers.

For this study, Harvard researchers at Dana-Farber Institute interviewed 603 patients and found 188 had discussed end-of-life care with their doctors. They found those patients were less likely to use more intensive interventions, and as a result, had lower medical bills. In fact, health costs for patients who had end-of-life conversations with their doctors were estimated to be an average of 36% lower in the last week of life than for patients who didn't talk with their doctors.

There was no difference in survival or in psychological distress between the two groups. However, patients who didn't have end-of-life conversations with their doctors experienced more physical distress in the final week of life, as reported by nurses and caregivers.

"We refer to the end-of-life discussion as the multimillion-dollar conversation because it is associated with shifting costs away from expensive, burdensome, non-curative care, like being on a ventilator in an intensive care unit, to less costly comfort care provided at home or in hospice, which most patients and their families say they would prefer," said Holly G. Prigerson, PhD, of the Center for Psycho-Oncology and Palliative Care Research at the Dana-Farber Cancer Institute and an author of the study.

Studies have shown that about one-third of Medicare expenditures in the last year of a person's life are spent in the last month. A high proportion of those costs come from life-sustaining procedures and care, such as ventilator use and resuscitation.

Promoting policies that increase communication between doctors and patients could lead to better quality-of-life and lower costs at the end-of-life for patients with advanced cancer, the researchers conclude.

In another study published in the same issue of the Archives of Internal Medicine, researchers analyzed data from 158,780 Medicare patients who died in 2001 and found that despite dying of similar causes, black and Hispanic patients appeared to have significantly higher end-of-life costs than white patients. Most of this was due to greater use of life-sustaining care and procedures. The researchers weren’t able to look at the reasons for these differences in the study.

"These reports help shed light on an important issue: differences between certain populations in end-of-life care, including lower rates of hospice use among certain populations," says Otis W. Brawley, MD, chief medical officer of the American Cancer Society. "But the issue here may not be about race as much as it is about economics and culture. People who do not have adequate access to health care often do not have an ongoing relationship with a specific doctor. Without a regular source of care, there's less opportunity to build a trusting relationship."

More research is needed, he says.

"These are important, intriguing findings, but it's important that other researchers replicate them before we draw further conclusions. Once that's done, we'll need to explore in more detail what factors may contribute and how. But this data clearly supports the need for further research."

WASHINGTON (CNN) -- An increase in the number of suicides among military personnel can be traced, in part, to a "stressed and tired force" made vulnerable by multiple deployments, a military leader said Wednesday.

"We must find ways to relieve some of this stress," said Gen. Peter W. Chiarelli, vice chief of staff of the Army, in testimony before the Senate Armed Services Military Personnel Subcommittee.

"I think it is the cumulative effect of deployments from 12 to 15 months," he said, adding that the longer deployments are scheduled to continue until June.

He cited long deployments, lengthy separations from family and the perceived stigma associated with seeking help as factors contributing to the suicides.

Adm. Patrick M. Walsh, vice chief of naval operations, said suicides are the third leading cause of death in the Navy.

"We must eliminate the perceived stigma, shame and dishonor of asking for help," he said.

Gen. James F. Amos, assistant commandant of the Marine Corps, said his branch of the service has incorporated education and training about suicide prevention "at all levels."

He said four of 55 mental health professionals deployed in the U.S. Central Command were recently embedded with Marines. He expressed optimism that that tactic would pay off, but he said he had no data to support his expectation.

And Gen. William M. Fraser, vice chief of staff of the Air Force, said his branch, too, was taking steps "to ensure airmen are as mentally prepared for deployment and redeployment as they are physically and professionally."

Sen. Lindsey Graham, R-South Carolina, said the efforts have not sufficed. He noted that last year, for the first time, the suicide rate among military personnel has exceeded that of the civilian population. "What's going on?" he asked rhetorically.

The panel members made clear that, whatever is going on, it is complex. Chiarelli said the Army tallied 133 confirmed suicides last year and is still investigating another seven possible ones. At least 70 percent of the suicides had "some kind of relationship problem," he said. Often, the situation was worsened by something else, such as a financial or legal problem, he said.

And it wasn't just the trauma of war that appears to heighten the risk. The suicides were about evenly divided among those who had returned from deployment, those who were still deployed -- some on a third or fourth tour -- and those who had never been deployed, Chiarelli said.

All of the military leaders said they had too few mental health professionals in their ranks.

But Walsh said professionals are not the only people who must be trained to intervene, noting that it is often the shipmate or the battle buddy who seeks assistance for someone in need.

And Navy counselors are now asking family members for feedback that often proves more illuminating than what the sailors themselves are willing to impart, he said.

"If I survey the family, I'm going to get a different set of answers."

One reason some service personnel are reluctant to seek help from their chain of command is because they fear they will then be passed over for promotions, the panel members said.

In some cases, they pay out of their own pockets to seek help privately, Rubenstein said.

And others call civilian hotlines, one of which reported getting three calls per day this year from active-duty military personnel.

Representatives of all of the branches of the military said they have seen recent increases in suicides.

At the Pentagon, Defense Secretary William Gates said he believes the 15-month deployments "were a real strain" on many, but expressed optimism that beneficial changes are in the works.

"All the services are are addressing this problem, but the Army in particular, I think, is really going after it in a very aggressive way," he said.

But some suicides appear to defy all prevention efforts. Maj. Gen. David A. Rubenstein, deputy surgeon general of the Army, cited the case of a 33-year-old soldier who was living at home with his wife and three children.

He suffered a traumatic brain injury more than two years ago and had been giving weekly motivational speeches to other soldiers in a clinic.

For the past two years, the soldier also had been seeing a psychiatrist. The most recent visit occurred last Friday.

On Monday, he saw his primary care doctor and his nurse case manager, and also had a life-skills appointment.

"On Tuesday, he apparently committed suicide," Rubenstein said. "This soldier was treated, compliant and supported in every way, and yet he's dead today."

The suicide underscores that suicide is "a complex, very difficult problem that causes all of us to scratch our heads and wonder: how do we stop the next one?"

Last year, at the U.S. Military Academy at West Point, two cadets and two members of the staff and faculty killed themselves, said Brig. Gen. Michael S. Linnington, commandant of the cadets.

He cited stress from broken relationships and, in one case, a pre-existing mental health condition that academy officials had not known about at the time of admission. None of the four had been deployed to a combat zone.

Linnington called the spate of self-directed violence at the school "troubling and unacceptable" and said preventive measures there had been beefed up in recent months.

Some of your male clients may think that Pilates is only a woman's workout – it’s not. Shrimp and Pesto Pizza
Makes 8 slices


With all its herbs, the pesto provides antioxidants—and a welcome break from plain-old-tomato sauce. The flax meal in the dough makes for a tasty, fiber-rich crust that boasts omega-3 fatty acids.

Shrimp and Pesto Pizza

Pizza

INGREDIENTS:
• Nonstick cooking spray
• 1 recipe whole-wheat and flax pizza dough (recipe follows)
• 1/4 cup reduced-fat pesto
• 6 oz large raw shrimp, peeled and deveined (approx 20 to 24 shrimp)
• 1/2 medium-sized red bell pepper, sliced

INSTRUCTIONS:
Preheat oven to 450 degrees Fahrenheit. Set baking rack in top third of oven. Lightly spray a baking sheet with nonstick cooking spray. Shape dough into a 12-inch round pan on prepared baking sheet. Spread pesto over dough. Top pizza with shrimp and pepper. Bake for 12 to 15 minutes. Cut into 8 slices and serve.

Whole-wheat and Flax-Meal Pizza Dough

INGREDIENTS:
• 2 1/4 cups whole-wheat flour
• 1/4 cup flax meal
• 1 tsp instant-rising yeast
• 1 tsp sugar
• 1 tsp salt
• 1 cup warm water (100 to 110 degrees Fahrenheit)
• 1 tbsp extra-virgin olive oil

INSTRUCTIONS:
In a large mixing bowl, combine flour, flax meal, yeast, sugar and salt. Stir in water and oil with a wooden spoon. When the dough starts to hold together, remove from bowl and knead by hand for 8 minutes (a standard mixer fitted with a dough hook makes this process much easier). Shape dough into a ball and return to bowl. Cover bowl with plastic wrap and let dough rise for one hour in a warm spot (70 to 80 degrees). Before baking, remove and knead dough for 5 or 6 strokes, then shape.

Calories: 201, Total Fat: 6 g, Sat. Fat: 1 g, Carbs: 28 g, Fiber: 6 g, Sugars: 1.5 g, Protein: 11 g, Sodium: 386 mg, Cholesterol: 34 mg

Fuel your body right with lots of power-packed grilled chicken. Make double the amount and freeze the extras.


Grilled Chicken With Mushrooms


Ingredients:

• 3 to 4 ounces skinless chicken breast
• KC Masterpiece Hickory Smoke Marinade (or your favorite marinade), to taste
• McCormick’s Mesquite Chicken Seasoning, to taste
• Cooking spray
• 8 ounces pre-sliced mushrooms
• Soy sauce, to taste
• 1 slice soy cheese
• 1 whole wheat hamburger bun

INSTRUCTIONS:

Brush both sides of the chicken with marinade and sprinkle it with mesquite seasoning. Turn the grill on high, and cook for three minutes. Now, turn the chicken over, cover it with foil, and reduce the heat to medium-high. Continue to turn it (every two to three minutes), and gradually reduce the heat. Keep turning it until the chicken is almost done.

While the chicken is cooking, spray a medium-sized pan with cooking spray, and heat it on high. Add in the mushrooms, drizzle in the soy sauce. Stir and cover. Continue to stir frequently, and reduce the heat to medium. Remove mushrooms when they are tender.

Next, place the cheese on the bun and put it into the toaster oven. Heat it until the cheese melts. Finally, place the chicken on the bun, smother it with the mushrooms, and serve.

New Recipe Of The Week







Feel like adding to your eating plan? Easy and yummy, this is for you.

Asparagus and Cheese Omelette




Ingredients:

• 4 egg whites
• 1 egg yolk
• 1 tablespoon onion, finely chopped
• 1 tablespoon skim milk
• 5 spears asparagus
• 1 ounce Swiss cheese
• Pepper to taste
• Cooking spray

Instructions:

In a nonstick frying pan coated with cooking spray, cook the onion until it’s clear. In a bowl, beat the eggs together with the milk, and then add it to the onions. Once it’s cooked on that side, flip it over, and then add the asparagus and cheese to the upright side of the egg. Once the egg is completely cooked, fold in half to cover the asparagus, and let it cook for another couple of seconds. Serve immediately.

Lime and Cilantro Fish Marinade



If healthy eating doesn’t come naturally, try something different: a savory marinade.

Ingredients:


• 4 limes, sliced and seeded
• 1 bunch cilantro, chopped
• 1/2 red onion, chopped
• 4 cloves garlic, crushed
• 3 jalapenos, seeded
• 3 tablespoons flaxseed oil
• 1/4 cup water
• 2 tablespoons oyster sauce
• 1 tablespoon Mrs. Dash
• 8 or 9 white fish fillets

Instructions:


Blend all of the ingredients together, except the fish, for 30 seconds. Then pour the mixture over the fish fillets and let them marinate in the fridge overnight in a covered container. Remove the fish from the sauce and broil it in the oven for two minutes on each side. Serve immediately.

After maintaining an austere, healthy diet for a few weeks, the desire to eat a juicy cheeseburger proves too irresistible for some dieters.

"It's natural that most people when they are following a New Year's resolution or any program to change something in their life, that they will fall off the wagon or they'll have a setback or a failure," said Jillian Michaels, a personal trainer for NBC's show "The Biggest Loser."

Rather than beating yourself up after having a setback, learn from the missteps, experts say.

It's a matter of attitude, Michaels said. Learn from the setbacks and view "a failure as a point of learning, an opportunity to see what works and what doesn't."

"We eat with our eyes, not out of our stomach," said Dr. Melina Jampolis, the diet and fitness expert for CNNhealth.com. "It's much more difficult to control serving sizes."

People have a tendency to keep eating until their plate is clean or the container of cookies is empty.

"A lot of people eat too much and their clothes feel tight. You shouldn't have to unbutton your pants, blouse or unzip anything after your meal," said Cheryl Forberg, the nutritionist for "The Biggest Loser." "You lose sight of your body's natural hunger cues."

To become more attuned to your body, avoid eating while standing up, sitting at the desk or inside a car, she said. Dedicate time for meals and snacks.

Rather than viewing meals as an obligatory part of the day, "learn to enjoy the process. At the same time, you begin to stop eating when you're comfortable, instead of when you have to unzip your jeans," Forberg said.

Avoid stocking problem foods like cookies or ice cream in the house if you know you can't resist them.

"When you know you have a weakness with chocolate or cookies, you probably shouldn't keep it in the house if you know you can't stop at one," she said.

One way to retrain the body is to divide snacks by portion sizes.

"Those 100-calories packs retrain people to the eye and decrease the snacking," Jampolis said. "It's more expensive and not environmentally friendly, but you're much better off getting a 100-calorie pack rather than eating out of container."

"If you're on a budget and you want to be more environmentally friendly, make your own. Every Sunday, measure 100 calories into seven Ziploc bags," she said. You can even recycle the bags.

The Purpose Of Nutrient Timing


You’ve probably heard that breakfast is the most important meal of the day. While this is difficult to refute, pre- and post-workout nutrition are tied for a close second, with peri- nutrition just a nose behind. Refueling immediately post-workout is probably not a new concept. Working out depletes glycogen (the storage form of carbohydrate) and promotes protein breakdown (catabolism). Eating the proper nutrients soon after a workout will help replenish this glycogen and enhance protein building (anabolism). Always remember that working out is merely a stimulus needed to enhance muscle growth and recovery. However, it’s the actual time between workouts when your muscles grow.

The late Mike Mentzer always used a great analogy when discussing training. Lifting weights is like digging a hole in your muscles: if you continue to train and train without allowing for adequate recovery, the hole, per se, will only get bigger. Although he was speaking more in terms of overtraining, this same philosophy can be adopted to nutrition.

If you continue to train and don’t feed your body the nutrients it needs (adequate energy via high nutrient macronutrients), the hole will continue to grow deeper. Refueling the body allows for growth (i.e., you fill the hole from training with new lean body mass). Ultimately, over time this hole will not only “fill in” but will begin to overcompensate by overflowing or growing larger by building more lean body mass. Make sense? Extrapolating this example to nutrition and what you now know happens during rest, you should be well-aware that refueling your body around your workouts is crucial for recovery and optimal performance. Since this is not a new discovery, this article will concentrate more on pre- and post-workout nutrition. Several recent studies have demonstrated the importance of this concept, so let’s take a look.

As I mentioned, training results in muscle protein breakdown. Therefore, it makes sense that the higher the baseline protein status, the less negative impact training would have. To look at this simplistically, if you start at 100 percent protein status and go down to 50 percent from training, it would be better than starting at 50 percent and going down to 0 percent from training. In the second example you would always be trying to get up to 100 percent, whereas if you consistently fed your body the nutrients it needs, you would be better off.

A recent study investigated whether ingestion of a supplement (six grams of essential amino acids and 35 grams of sucrose) taken immediately before or after a training bout would alter the net protein balance in muscle. Interestingly, the authors noticed a significantly greater increase in those taking the pre-workout supplement compared to those taking a post-workout supplement. It appears that the mechanism here is there were more amino acids (remember, these are the building blocks of protein) available for the working muscle. Moreover, this effect carried over so that there was enhanced availability of amino acids for at least the first hour of the workout. So, you ask, “What if my workouts last more than an hour?”

Well, maybe it’s time to refeed your body some more nutrients during your workout. Why should you hinder your workout because your muscles are tired, hungry and just plain beat up? Race car drivers have the fastest, top notch cars available. However, during the Indy 500, they need to stop to refuel. Think of your muscles as race cars. If you don’t give them the fuels they need, they won’t perform.

So now the question is not only if you should feed your body before and/or during a workout, but what should you feed your body at these times. As I discussed in the previous study, participants received six grams of essential amino acids (equivalent to approximately 12 to 15 grams of whole proteins) and 35 grams of sucrose (carbohydrate). In terms of protein, there are obviously a million choices. But I don’t think gnawing on a filet mignon on your way to the gym is intelligent. As you are all aware, there are also a number of protein powders available (i.e., whey, casein, soy, etc). Furthermore, the processing of these proteins differs too (hydrolysates, isolates and concentrates). Wow, this is starting to get a bit technical. Let’s cut to the chase.

Studies have shown that whey protein, when consumed independent of any other foods, is absorbed more rapidly than casein protein. Although most of the time foods are often combined with one another, it’s safe to say whey is generally absorbed more rapidly. In terms of the various processing methods, taking one over the other will not make or break your muscle gains. Therefore, if taking a protein supplement prior to a workout, it would be best to take a quality whey protein supplement. Similarly, if taking a product during or after a workout, whey would take the cake. This is because you want a protein that’s rapidly available to your muscles. You don’t want to be sitting at the gym with a stomach full of protein powder sloshing around. Try a set of squats like that! With that said, mixing whey protein with some carbohydrates (either via powder or your favorite carbohydrate product) would be wise.

There are a number of pre-designed formulas on the market. In general, it would be best to stick with a carbohydrate:protein ratio of approximately 3 or 4:1 (e.g., for every 30 or 40 grams of carbohydrate, you should have 10 grams of protein). This could be a great thing to drink slowly on the way to the gym and continue to sip throughout your workout. Then, be sure to consume sufficient energy immediately after your workout and continue to refeed your body throughout the day. Of course, whole foods provide more nutrients than any supplement can provide, but carbohydrate:protein supplements are much more applicable and easily digested, so they are useful immediately before, during and after workouts.

So now let’s get back to the frequency of eating statement from the beginning. Utilizing the recommendations from my last piece on meal frequency, plus those in this column, one would be eating about eight times per day. Without getting into meal specifics, here’s an example of a day:

* Meal 1 - Breakfast
* Meal 2 - Pre-workout drink
* Workout
* Meal 3 - During workout drink
* Meal 4 - Post-workout drink
* Meal 5 - Lunch
* Meal 6 - Afternoon snack
* Meal 7 - Dinner
* Meal 8 - Evening snack

Of course, when I use the word “snack,” I am referring to a nutrient dense food, such as low-fat yogurt, fruit, etc. rather than what most Americans consider a snack. As long as your energy intake is sufficient for muscle growth and enhanced recovery, but not so high it’s resulting in fat storage, you’re on the way to a new you. The best judge of that is the mirror, not the scale or anyone else. Only you can set goals for yourself and shoot to achieve those. Don’t let anyone or anything get in your way.
